Thank you for choosing Justice Polygraph Services to advise and implement you about Polygraph Examination procedures. We are excited to work with you to obtain the truth about a known issue or a known allegation.

Justice Polygraph Services shall perform the Consultancy Services at the time and in the manner specified in this Agreement with due care, skill, and judgement in accordance with the highest professional standards to the American Polygraph Association, Association of Police Polygraphists, and the Florida Polygraph Association.

IN CONSIDERATION OF the matters described above and of the mutual benefits and obligations set forth in this Agreement, the receipt and sufficiency of which considerations is hereby acknowledged, the Client and Justice Polygraph Services (individually the "Party" and collectively the "Parties" to this Agreement) agrees as follows:

3.

Background

A. The Client is of the opinion that the Consultant has the necessary qualifications, experience and abilities to provide consulting services to the Client.

B. The Consultant is agreeable to providing such consulting services to the Client on the terms and conditions set out in this Agreement.

4.

Services Provided

  • —Recommendations to client regarding the known issue(s) or the known allegation(s).
  • —If Client(s) agrees to conduct Polygraph Examination, Test Questions Composition will be made.
  • —If Client(s) agrees to conduct Polygraph Examination, Scheduling Polygraph Examination date.
  • —Creation of Intake Polygraph Examination file.
  • —Obtain disclosed information from client.
5.

Client Requirements

  • —Client agrees to follow the terms and conditions of the agreement.
  • —Client agrees to pay for the consultation fee(s).
  • —Client agrees to be truthful about the information that he/she is disclosing.
  • —Client agrees to a full disclosure of the known issue / known allegation.
6.

Terms of Agreement

The term of this Agreement (the "Term") will begin on the date of this Agreement and will remain in full force and effect until the end of the consultation, subject to earlier termination as provided in this Agreement. The Term may be extended with the written consent of the Parties.

7.

Performance

The Parties agree to do everything necessary to ensure that the terms of this Agreement take effect.

8.

Compensation

  • —Justice Polygraph Services will charge the Client a flat fee of $150.00 for the consultation (the "Compensation").
  • —The client will be invoiced for the services provided.
  • —Invoices are to be paid in full prior to any services rendered by Justice Polygraph Services.
  • —In the event that this agreement is terminated by the Client prior to the completion of the Services but where the services have been partially performed, Justice Polygraph Services will be entitled to pro rata payment of the compensation to the date of termination, provided that there has been no breach of contract on the part of Justice Polygraph Services.
  • —The Compensation as stated in this Agreement does not include sales tax, or other applicable duties as may be required by law. Any sales tax and duties required by law will be charged to the Client in addition to the Compensation.
9.

Reimbursement of Expenses / Refund Policy

Justice Polygraph Services has a non-refundable policy due to scheduling and time consumption.

10.

Confidentiality

  • —Confidential Information (the "Confidential Information") refers to any data or information relating to the Client, whether business or personal, which would reasonably be private or proprietary to the client and that is not generally known and where the release of the Confidential Information could reasonably be expected to cause harm to the Client.
  • —Justice Polygraph Services agrees that they will not disclose, divulge, reveal, report or use, for any purpose, any Confidential Information which the consultant has obtained, except as authorized by the Client or as required by law. The obligations of confidentiality will apply during the Term and will survive indefinitely upon termination of this Agreement.
  • —All written and oral information and material disclosed or provided by the Client to Justice Polygraph Services under this Agreement is Confidential Information regardless of whether it was provided before or after the date of this Agreement or how it was provided to Justice Polygraph Services.
12.

No Guarantee

Justice Polygraph Services does not warrant or guarantee any specific level of performance or results.
